STANDARD BOARDING AGREEMENT
1. Hassayampa Canine Resort & Spa
agrees to exercise due diligence in the care of my dog. In addition, Hassayampa
Canine Resort & Spa agrees to keep its premises sanitary and properly enclosed. My dog will be cared
for by Hassayampa Canine Resort & Spa staff only, without
liability on Hassayampa Canine Resort & Spa’s part
for loss or damage from disease, theft, fire, death, escape, injury or harm to persons, other dogs or property by my dog,
or from other unavoidable causes, due diligence and care having been exercised by Hassayampa
Canine Resort & Spa.
2. In an emergency, Hassayampa
Canine Resort & Spa will attempt to contact the Pet's personal veterinarian as well as the emergency
contact provided by me: however, such an emergency might not provide the time to do so prior to the administration of care.
I authorize Hassayampa Canine Resort & Spa to obtain medical
attention for my pet from any qualified veterinarian and to transport my pet to and from that veterinarian when Hassayampa
Canine Resort & Spa deems such medical care is important to my pet's health. I grant Hassayampa Canine Resort & Spa or its employees or agent’s full power
of decision-making involving the medical care or transport.
3. I agree that when I board two or more dogs
in a suite and permit them to play together that nicks and scratches may occur and that staff may not notify me immediately
if my pet sustains any nicks or scratches.
4. I hold Hassayampa Canine
Resort & Spa harmless in the event of injury or accident during my pet’s stay or transportation.
5. In the event of my pet’s death my emergency contact will be notified immediately. I expressly
agree that Hassayampa Canine Resort & Spa's liability shall
in no event exceed the lesser of the current chattel value of a pet of the same breed or the sum of $200.00 per animal admitted.
6. I affirm that my dog is in good general health. After close examination, dogs arriving with fleas and/or
ticks will be bathed at the owner's expense. I acknowledge and agree to assume all risk of illness, disease, harm or
otherwise to my pet by allowing my pet to stay at Hassayampa Canine Resort &
Spa.
7. I agree that my dog may be videotaped, photographed, and/or recorded. Hassayampa
Canine Resort & Spa shall be the exclusive owner of the results and all such tapings, photography,
and recordings will be used only by Hassayampa Canine Resort & Spa
in its own promotion, advertising, sale or publicizing.
8. Should my dog exhibit inappropriate aggressive behavior
toward other dogs or people, for the safety of all concerned he or she will remain in a secure dog run.
9.
I agree that in the event I do not pay charges due in accordance with this Agreement or if I abandon my pet, Hassayampa Canine Resort & Spa may exercise its lien rights within ten (10)
days after written notice has been given to me by Hassayampa Canine Resort &
Spa by certified mail to address shown on Agreement. Hassayampa
Canine Resort & Spa may dispose of pet for any and all unpaid charges, at private or public sale,
at the sole discretion of Hassayampa Canine Resort & Spa
and I specifically waive all statutory or legal rights to the contrary. If such sale shall not secure a price
adequate to pay such costs of pet care or other charges delinquent, plus costs of sale, I shall be liable to Hassayampa Canine Resort & Spa for the difference. All monies realized
by Hassayampa Canine Resort & Spa at such sale, over and
above the charges due and costs of sale, shall be paid to me.
10. By signing this Agreement and leaving my pet
with Hassayampa Canine Resort & Spa, I specifically represent
that I am the sole owner of my pet, free and clear of all liens and encumbrances. I certify to the accuracy of all information
provided to Hassayampa Canine Resort & Spa in writing and
orally about my pet. Hassayampa Canine Resort & Spa
reserves the right to deny admittance to my pet or any pet for any reason at any time.
11. I agree to be solely responsible for any and all
acts or behavior of my pet while in the care of Hassayampa Canine Resort &
Spa, to include payment of costs for injury to staff or other animals or damage to facilities caused
by my pet. I further indemnify Hassayampa Canine Resort & Spa
against any claims made against Hassayampa Canine Resort & Spa
or its employees or other agents or losses or damages of any kind suffered by Hassayampa
Canine Resort & Spa as a result of my failure to informHassayampa
Canine Resort & Spa of any pre-existing condition my pet may have (such as illness or aggression
problems) or which were otherwise caused by my pet.
12.
I agree to pay the pet care service rate and to pay for any additional services requested when I pick up my pet. I agree
that my pet shall not leave the facility until all charges are paid or both parties agree upon proper payment arrangements.
13. I understand that check-out time is 11:00 AM. I agree to pay a late charge of $20.00 per dog if I elect
to pick up my dog between 11:00 AM and 3:00 PM. I agree to pay a full day boarding fee if I pick up my dog after 3:00
PM.
14. I agree that if I fail to cancel my pet’s reservations 48 hours prior
to check-in I will be billed for one (1) night’s stay if it is not during a peak or holiday period. In addition, I agree
that if I have not cancelled my pet’s reservations two (2) weeks prior to check-in during a peak or holiday period,
I shall be charged for entire stay.
